The WISH project aims to demonstrate the feasibility of a new, more efficient \u201cout of hospital\u201d follow-up strategy for women with HER2-negative, ER+ early breast cancer who are receiving adjuvant hormonal treatment. At present, follow-up is managed by the hospital oncology team, which has limited capacity to address the complexity of this clinical process. WISH is expected to shift follow-up management from the hospital to primary care, through an efficient sharing network between the patient, the general practitioner and the oncologist.<\/p>\n

To demonstrate the efficacy of WISH-supported follow-up compared with standard care, the project has designed a multicentre, international, randomised trial. The primary endpoint is non-inferiority in quality of life at 12 months, assessed with the Functional Assessment of Cancer Therapy \u2013 Breast (FACT-B) questionnaire. Secondary endpoints are time to toxicity detection, time to intervention and cost-effectiveness. Patients will be randomised between WISH-assisted and hospital-based follow-up, according to the recruiting centre, type of surgery (breast-conserving or mastectomy) and urban or rural residence.<\/p>\n

The WISH app will provide a user-friendly tool to collect patient-reported outcomes and to manage adverse events promptly, strengthening cooperation among all the actors involved in follow-up. WISH will also allow personalised lifestyle interventions to improve patients\u2019 well-being and long-term outcomes.<\/p>\n

Remote monitoring will help to address health inequalities, by ensuring close follow-up of all patients, including those living in rural areas and those more likely to discontinue treatment. Data-driven analytics and advanced AI methods will help healthcare policy makers to improve forecasts of ill-health and to identify disease trajectories.<\/p>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n<\/section>\n
