For patients with severe resistant infections in Europe, the patient-centred pathway includes outpatient parenteral antimicrobial therapy (OPAT) at home, supervised by nurses (H-OPAT), which moves care from the hospital closer to home. However, for several antibiotics, H-OPAT still requires frequent hospital visits for traditional therapeutic drug monitoring (TDM), to keep drug levels within the target range. TDM causes logistical burdens and avoidable costs, and discomfort for patients, who must travel and undergo venipuncture. In addition, TDM-based dose adjustments are often an \u201ceducated guess\u201d, which leads to suboptimal antibiotic exposure and more healthcare needs.<\/p>\n

To make H-OPAT a truly de-institutionalised treatment without lowering the quality of care, a transformative solution is needed that improves antibiotic dosing and monitoring and removes the need for hospital visits. The AT-HOME consortium of academic hospitals, nursing organisations and industry, with the support of patients, will evaluate whether H-OPAT with vancomycin can become more efficient, effective and sustainable by integrating two elements:<\/p>\n
