05.The spectrum encompasses interventions and care models for individuals with type II diabetes mellitus, acute stroke, dementia, multimorbidity, severe mental illness, and institutionalised individuals; the prevention of adverse events in this population; the development and analysis of services for individuals with multimorbidity (case management, transitional care, etc.); and the health impacts of the role of caregiving in family members of these patients in the medium- and long-term, among other aspects
